Factors that Predict Doximity Rankings of Anesthesiology Residency Programs

Background: A total of 37,103 applicants participated in the Main Residency Match in 20181. Of those, 2,004 applicants applied for positions in anesthesiology residency programs. Perceived prestige of a residency program has consistently been shown to be an important factor for applicants deciding on their rank order list2. Doximity’s Residency Program Navigator (RPN) is a widely used ranking system for residency programs of many different specialties. The majority of applicants to residency consider Doximity’s reputation rankings "valuable" or "very valuable” and report that these rankings influence their approach to applying to residency and making rank order lists3.

Doximity’s rankings are derived from reputation data that comes from survey responses of board-certified physicians in a given specialty4. It is unclear what factors influence reputation data scores. The aim of this study is to assess factors that may predict Doximity’s rankings of anesthesiology residency programs.

Methods: To determine predictive factors for Doximity’s reputation rankings, a logistic regression was run with the following independent variables: Total number of residents in a program, program age, total funding from the National Institutes of Health (NIH) awarded in 2018, and US News & World Reports (USNWR) ranking of the associated medical school.

Reputation rankings, program size, and program age for anesthesiology residencies were collected from Doximity’s 2018-2019 RPN. Data for departmental funding from the NIH were collected from the Blue Ridge Institute for Medical Research’s 2018 funding report. Ranking data for medical schools were collected from USNWR 2018-2019 report.

Results: A multivariable linear regression revealed statistically significant correlations between Doximity ranking and each predictor variable (Table 1, adjusted R2 = 0.713, p<0.0001). An increase in program size by one resident is correlated with an improvement by nearly one rank on Doximity (Figure 1).

Conclusions: Doximity rankings of anesthesia programs appear to be predicted by residency program size, age, amount of NIH funding, and whether the associated medical school is ranked on USNWR. It is unclear whether Doximity’s reputation ranking algorithm favors larger programs that inherently have more potential survey respondents than smaller programs. In private email communication, Doximity did not answer this question. More transparency in Doximity’s ranking process is warranted. Applicants to anesthesiology residency should critically assess the value of Doximity’s reputation rankings in the context of other factors when making decisions about where to apply.
